The post is based in Bath and North East Somerset, a UNESCO World Heritage Site. It is a beautiful city, surrounded by countryside, offering a rich arts, culture, and sports scene. It is easily accessible from surrounding areas, with rail links to Bristol and beyond. Staff parking, pool cars, electric charge points, and secure cycle storage are available.

Main duties of the job
You will join the team at a time when the key worker role (replacing care coordination) has been introduced. Your responsibilities will include providing targeted interventions to individuals with significant mental health needs and those living with dementia, aiming to enhance wellbeing and promote independence. This involves working collaboratively in a holistic, solution-focused manner, delivering therapeutic interventions, supporting treatment engagement, assessing and managing risks, evaluating care needs, and assisting in the development of appropriate care and support plans.
The role offers opportunities to develop supervisory skills, including supervising both qualified and unqualified team members.
