The role involves working with experienced Community Nurses, partnering with stakeholders and voluntary services to deliver the best community care and shape future healthcare in Milton Keynes.
We seek nurses who can think quickly and handle challenges proactively. Join our motivated team to develop our service beyond traditional nursing roles, addressing national healthcare challenges.
The working hours are 9 am – 5 pm, 7 days a week, including bank holidays, with plans to extend to 9 am – 8 pm. A driving license and car are essential.
Main duties of the job
* Demonstrate professional and clinical knowledge through Level 1 registration.
* Develop practical nursing skills with accuracy and dexterity.
* Work with the District Nurse to provide skilled patient care at home or other settings.
* Work independently, prioritize workload, and manage changing demands.

* Provide skilled nursing care in partnership with the District Nurse and GPs.
* Undertake delegated duties under supervision.
* Manage caseloads, including new visits, assessments, and referrals.
* Liaise with the primary care team and social care services.
* Follow infection control policies and promote self-care among patients.
