Position
Blackpool Community Mental Health Team is seeking to appoint a Band 5 Community Mental Health Nurse/Social Worker.
Responsibilities
* Deliver holistic care to patients in a community setting, including homes and clinics.
* Manage travel requirements to provide a comprehensive service.
* Provide high-quality, patient-centred care, promote independence, and support patients with a wide range of health needs.
* Develop a broad and valuable clinical skill set, managing diverse conditions and working autonomously while being supported by a multidisciplinary team.
