We are seeking experienced, compassionate, and organized individuals to provide high-quality, recovery-focused support to clients with mental health needs, including learning disabilities, physical health issues, and substance misuse, while promoting independence and social inclusion.
Job Purpose: To manage teams within MHM's Community Matters services, ensuring high-quality service delivery to clients.
Duties include:
1. Deliver emotional support, advice, and guidance via face-to-face, telephone, and video interactions.
2. Assess situations to determine risks and clients' needs.
3. Make decisions under pressure while communicating effectively.
4. Accurately record contacts, visits, and calls on electronic systems.
5. Work in line with performance targets and contractual requirements, providing data and case studies as needed.
6. Participate in individual risk assessments and reviews.
7. Develop and implement action and safety plans to meet individual needs.
8. Encourage clients to recognize, understand, and manage factors affecting their mental well-being and physical health.
9. Collaborate with partners and external agencies to meet clients' needs and ensure safety and service delivery.
