Adults Senior Practitioner - Preparing for Adulthood (PfA) Team
Hackney Council is looking for an experienced Senior Practitioner to join the Preparing for Adulthood (PfA) team on a locum basis, starting immediately. This is a fantastic opportunity to support young people with disabilities and complex needs as they transition into adulthood.
Role Overview
Manage a complex and varied caseload, including high‑risk and statutory cases
Complete Care Act assessments, contribute to EHC plans, and lead on personalised support planning
Provide mentoring, guidance, and practice leadership for Social Workers, NQSWs, and students
Support the management team with supervision, audits, and case allocation
Work collaboratively with health, education, housing, and provider partners
Produce high‑quality reports for court and multi‑agency meetings
Ensure practice is fully compliant with the Care Act 2014, Children Act 1989/2004, MCA, and all statutory requirementsRequirements
Social Work qualification (Degree/DipSW/CQSW)
SWE registration
Strong background in children's disability services or adult learning disability services (PfA)
In‑depth knowledge of the Care Act, MCA, and children's legislation
Excellent risk assessment, analytical, and decision-making skills
Experience as a Practice Educator or mentoring staff
Ability to travel for home visits and work in a hybrid modelWhat's on Offer
