Weare seeking a motivated and patient-focused Clinical Pharmacist to join our dynamic multidisciplinary (MD) team at Westbourne Medical Centre. This is an exciting opportunity for an experienced pharmacist to play a key role in delivering high-quality, safe, and effective patient care within a very supportive modern general practice environment.

As a Clinical Pharmacist, you will work closely with GPs, nurses, and other healthcare professionals to optimise prescribing, improve patient outcomes, and promote evidence-based practice. The role offers a diverse and rewarding workload, including medication reviews, managing long‑term conditions, supporting medicines optimisation, and contributing to practice development.

We offer a competitive salary, commensurate with experience, for a part‑time role of 18 to 20 hours per week with a minimum requirement of Wednesday mornings and flexibility for the remaining hours.

Main duties of the job

Todeliver a high standard of patient care as a Clinical Pharmacist in general practice, using advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base.

Tobe an expert in prescribing and medicines optimisation within the practice, delivering telephone consultations as part of the duty team and answering all medicines‑related queries from the MD team.

Maintain a flexible approach in collaboration with the GPs and other health professionals to provide comprehensive healthcare sensitive to the needs of the local population.

Tomanage own clinical workload in general practice, working within their own professional boundaries, responding effectively to patient need and ensuring patients’ choice and ease of access to services.

Tocarry out all duties in a professional manner and adhere to all protocols, policies and guidelines.

Job responsibilities

Duties and Responsibilities of the Post

Clinical Role

  • As a prescriber, to prescribe safe, evidence‑based, cost‑effective, appropriate medication and individualised patient care as defined by current legislative framework.
  • Promote and help compliance with the Dorset Formulary.
  • Refer patients directly to other services/agencies as appropriate.
  • Work with the Multi‑Disciplinary Teams within the practice to promote integrated and seamless pathways of care.
  • Be prepared to embrace templates, IT and all computer services to update knowledge.
  • Develop bespoke integrated pharmaceutical care plans for individual patients.
  • Engage with care homes to improve prescribing to residents.
  • Work with nursing, medical staff, health care assistant colleagues to ensure that Quality Outcomes Frameworks are being delivered.
  • Liaise with hospital pharmacy teams and specialists where appropriate, especially when completing medicines reconciliation from discharges.
  • Manage patients with more complex long‑term conditions. This will involve direct patient contact in the pharmacist's own clinic sessions.
  • Manage repeat prescribing requests, and work to rationalise and improve repeat prescribing systems.
  • Offer a holistic approach to the use and understanding of medicines by patients that includes step‑up and step‑down management required in Long Term Conditions and End of Life.
  • Facilitate access to medicines (e.g. sorting out secondary care referrals for specials, unusual formulations etc.).
  • Interpret national and local commissioning decisions as they relate to medicines.
  • Identify opportunities for savings and efficiencies where that does not conflict with the interests of individual patients.

Professional Role

  • Promote evidence‑based practice through use of the latest research‑based guidelines.
  • Support excellence and innovation in care through clinical audit, quality improvement and clinical research where appropriate.
  • Monitor the effectiveness of their clinical practice through quality assurance strategies such as the use of audit and peer review.
  • Maintain their professional registration.
  • Participate in continuing professional development to ensure that up‑to‑date evidence‑based knowledge and competence in all aspects of the role is maintained.
  • Record accurate consultation data in patient records in accordance with the latest guidance and other pertinent standards.
  • Actively take part in mandatory and further training as requested.
  • Adhere to practice guidelines re confidentiality.
  • Follow all practice Health and Safety guidelines and policies.

Person Specification

Qualifications

  • 5+ years post‑qualification experience working with patients from a practice, hospital or community setting
  • In‑depth knowledge of medicines and applied therapeutics
  • UK Registered pharmacist
  • Certificate/Diploma in clinical pharmacy
  • NHSE / CPPE Primary care pharmacy education pathway (PCPEP) Course

Experience

  • Minimum of five years experience post‑qualification
  • Experience of multidisciplinary team working
  • Experience of mentoring and training others
  • Experience of writing and reviewing policies / procedures
  • Experience of general practice

Knowledge and Skills

  • General knowledge of primary care and secondary care working
  • Ability to gather information on unfamiliar topics and disseminate to others.
  • Excellent written and oral communication skills
  • Ability to work alone and in a team
  • Demonstrates ability to organise and prioritise own workload and others in a team under pressure
  • Ability to implement risk management procedures
  • Ability to work developmentally
  • Ability to work with other professionals and influence decisions
  • Ability to reflect on own work and critically assess own work
  • Ability to make good clinical decisions
  • In‑depth knowledge of current issues in primary care services
