Band 6 Health and Justice Vulnerability Service - Adult Outreach

Are you an experienced Healthcare professional registered with a professional body looking for a new and exciting challenge working with individuals who have come into contact with the criminal justice system? If so, this job could be perfect for you!
We are looking for experienced, versatile, motivated, qualified nurses, social workers or allied health professionals with post-qualification experience to deliver a service to vulnerable individuals who are often the most marginalized within society. You will offer them hope and support in reducing their health inequalities and support with diversion away from the criminal justice system.
Main duties of the job
Be a suitably qualified professional (Nurse, Social Worker, Occupational Therapist, Speech and Language Therapist or other allied health professional)
Extensive post-qualification experience working in a range of settings, including community work, of at least 12 months
Excellent communication skills
Ability to work collaboratively and in partnership with agencies such as Police, Probation Services, Drug and Alcohol Services and many more!
To demonstrate sound assessment skills including identifying risk and informing and promoting decision making around complex care or safeguarding needs (mental health, exploitation and abuse, substance misuse, homelessness, learning disability)
To manage a caseload of adults referred to the service, empowering them to access the support to improve their lives.
To be a strong team player and able to work autonomously
To work in partnership with service users and their families/carers with regards to decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions.
Our team aim to:
* Improve access to healthcare and support services for vulnerable individuals and a reduction in health inequalities
* Divert individuals, where appropriate, out of the criminal justice systems into health, social care or other supportive services
* Reduce re-offending or escalation of offending behaviours

Person Specification
Qualifications
* Current Professional Registration - HCPC, RLDN, RNM, SWE
Experience
* Experience of working with mental health as a qualified professional
* Experience in independently assessing risk
* Experience in caseload management
* Experience working collaboratively with other agencies or professionals
* Extensive post-qualification experience within community and/or forensic services
Knowledge
* Experience supporting junior members of staff
* Knowledge of screening/triaging referrals
