Job Summary
To work as a member of the Intermediate Care Team at Central Manchester, providing direct care for patients in the community who have been referred to the service. The team is multi‑professional and multi‑agency, delivering assessment, rehabilitation and support to maintain people in their own homes, prevent re‑admission to hospital, and avoid inappropriate admissions to residential or nursing homes. Care is delivered in a variety of settings including units and patients’ own homes.
Responsibilities
* Follow agreed protocols for ensuring appropriate and timely input and maintaining pathways of care.
* Communicate with all agencies working with the service to ensure continuity of care while respecting patient choice and autonomy.
* Support the work of physiotherapists and occupational therapists in a community and bed‑based setting.
* Travel independently within the local geographical area to provide care in people’s homes and within the bed base.
* Work weekends and late shifts on a rota basis, with late shifts from 12 noon until 8 p.m.
* Deliver care in dedicated Intermediate Care beds, both nursing and residential, within Delamere House and Willow Bank (located in Withington) as the unit moves.
Qualifications and Experience
* Experience working in the NHS.
* Desirable: experience working in a rehabilitation setting.
