Job overview

A staff nurse position is available to join our Hospital at Home/Virtual Ward team within Central Lancashire. This service provides patients with safe, hospital-level care in their own homes or place of residence, helping to prevent unnecessary hospital admissions and supporting earlier discharges where appropriate.

Working in collaboration with Lancashire Teaching Hospitals and Lancashire and South Cumbria Foundation Trust, the service takes a multidisciplinary approach. Patients are reviewed by the clinical team through both face-to-face visits and remote monitoring, including telephone consultations and technology-enabled care.

The service is based at Leyland House, with visits carried out in patients’ homes, places of residence, or within the hospital to support onboarding to the service. It currently operates 7 days a week from 8am to 6pm, with plans to extend to 8am to 8pm.

A valid driving licence and access to a vehicle during working hours are essential, as the role involves visiting patients in their own homes.

Main duties of the job

We are seeking an enthusiastic, engaging, and caring individual to join the Central Lancashire team, with a passion for patient-focused care and a commitment to supporting and contributing to this developing service. The role requires someone who can work effectively both within a team and independently, with strong organisational skills to prioritise workload efficiently. Responsibilities will include carrying out patient assessments and treatments in their own homes, conducting remote monitoring and telephone assessments, and providing clear communication to patients, families, carers, and the wider multidisciplinary team. You will be expected to recognise changes in patients’ conditions and escalate appropriately, while also delivering education and support to help patients stay healthy for longer. In addition, you will play an active role in supporting learners and less experienced staff, maintaining accurate records, and contributing to daily huddles, departmental meetings, and MDT discussions. A commitment to personal development is essential, ensuring that practice reflects individual needs while working within local and national guidelines, processes, and policies.

Detailed job description and main responsibilities

· Assess, plan, implement and evaluate care to meet patients’ needs in accordance with agreed local and national standards and evidence-based guidelines

·Support and provide patient education and health promotion.

·Be responsible for the delivery and co-ordination of care, through appropriate professional practice, delegation and supervision of duties carried out by junior staff.

·Proactively manage risk and maintain patient safety, conforming to health, safety and security legislation, policies, procedures and guidelines.

·Assist in setting, monitoring and maintaining standards of care to improve the patient experience and outcome of care.

·Communicate all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamless care

·Maintain accurate and complete records of all care activities, in a timely manner, adhering to local and professional standards for documentation

·Act as a role model, presenting a positive image of self, team and the organisation

·Promote equality in care and practice by recognising, respecting and meeting the needs and choices of individuals

·Ensures environment of care is safe, well equipped and compliant with standards for hygiene and cleanliness.

·To participate in education and training of junior staff.

·Maintain and develop own professional competence and seek to extend the scope of personal knowledge and skills within scope of role and professional practice

·Proactively engage with Trust wide or local changes and developments in nursing practice e.g. intentional rounding, alcohol screening

·Actively try to resolve issues and complaints at local level and then report the outcome to line manager.

Person specification

Qualifications & Edcuation

Essential criteria

1. Appropriate NMC registration
2. RN1, RN2 General nursing posts
3. NMC recognised mentorship qualification or willingness to work towards following 18 month commencement in post
4. Evidence of continuing education

5. Degree

Knowledge & experience

Essential criteria

6. Willing to expand clinical role eg IV additives, venepuncture and cannulation
7. Awareness of changes within Nursing practice
8. Knowledge of Clinical Governance and the relevance to practice
9. Knowledge of relevant policies and procedures

Desirable criteria

10. Knowledge of ethical issues
11. Evidence of clinical competence in area of speciality eg surgery
12. Knowledge of Leadership and Management
13. Research and Development
