Role Overview

We're seeking to recruit new Community Enablement Workers—people with the right values and a positive attitude to work within the Specialist Support for Younger Adults team.

Our customers are aged between 18‑65 and have a range of physical and learning needs. We support them with personal care, manual handling, medication, food and drink, finances and shopping. The purpose of this job is to provide high quality support and care to customers in their own homes within a wider community, enabling them to remain independent while supporting their wellbeing, rights and choices.


Principal Responsibilities

* Support people within their own home to live an independent life.
* With reference to the individual's care plan provide practical and emotional support to customers to help them maintain independent living.
* Assist customers with physical disabilities to carry out activities, hygiene routines and manage medication as directed within their care plans.
* Assist customers in undertaking practical tasks to maintain their physical wellbeing and/or improve their living conditions.
* Promote independence through providing information to customers and liaising with carers, internal and external agencies. Support community engagement, budgeting, social activities and future goal planning.
* Update daily record sheets, communicate with outside agencies and ensure changes to a customer's needs are documented and reported so the care plan remains a live document.
* Undertake risk assessments, manual handling and medication assessments to safeguard customers and staff and meet service standards.
* Regularly review customer progress, monitor effectiveness and efficiency of the support plan, and document any necessary adjustments.


Qualifications & Experience

* Satisfactory clearance of an enhanced Disclosure and Barring service check.
* GCSE or equivalent.
* Care Certificate (or willingness to work towards).
* Level 2 or above in Health and Social Care or equivalent (or willingness to work towards).
* Proven experience in health or care work.
* Experience of delivering care services in a community‑based setting.
* Demonstrated interest in working in this area and the ability to undertake the role.
* Good verbal and written communication skills; ability to write clear, concise, and accurate reports.
* Excellent interpersonal skills with the ability to listen and interact effectively with and on behalf of customers, and to react appropriately to a crisis situation.
* Practical approach to problem solving and achieving results.
* Full UK driving licence.
* Flexible, adaptable, and tolerant to meet the needs of the service.
* Ability to meet the physical requirements of the role.
* Ability to attend, successfully complete and understand the need for mandatory training.
* Ability to inform customers about benefits and application systems.
* Ability to work within set procedures and systems.
* Demonstrated awareness and understanding of equal opportunities and other individuals’ behaviour, physical, social and welfare needs.
* Understanding of equality standards and diversity issues and their impact on social care services.

Working Patterns

* Weekends only (Saturdays and Sundays every week – 7am to 3pm or 3pm to 11pm).
* Day and Evening Shifts throughout the week (7am to 3pm or 3pm to 11pm).
