Join to apply for the ARRS Specialist Mental Health Practitioner role at Dorset HealthCare University NHS Foundation Trust. This is a permanent position to work in the Shore Medical Primary Care Network, comprising GP Practices based in east Dorset. The successful candidate will be an experienced Mental Health Practitioner with experience in a Primary Care and/or Community Mental Health Team setting. This role will work collaboratively between Primary Care and Secondary Mental Health Services as part of an Integrated Model of Care to support local communities and GP practices.
The practitioner will facilitate onward access to other secondary care-based services, third sector services, and link with existing support services via the Primary Care Network. The aim is to focus services around local communities and rebuild and reconnect healthcare teams to meet population needs.

This post is full-time 37.5 hours; however consideration will also be given to a job share or part-time working.
