Responsibilities
The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist develops a treatment plan, performs interventions to improve and enhance the patient's well-being, and evaluates progress.
* Evaluates the patient's functional status and needs, assisting the physician in developing the therapy plan of care.
* Observes and reports the patient's response to treatment and any changes to supervising nurse and/or physician.
* Treats patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice.
* Instructs and informs the healthcare team, patient, and family/caregivers about the plan of care, functional limitations, and progress toward goals.
Education and Experience
Licensure Requirements
* Current Physical Therapy licensure in Illinois.
* Current CPR certification required.
* Must have a valid driver's license, vehicle insurance, and access to a dependable vehicle.
