An exciting opportunity has arisen for a Band 6 Community Learning Disability Nurse to join the Cardiff Community Learning Disability Team.

The Registered Nurse will have an understanding of the knowledge and skills of the professional activities within a Community Support Team within the field of Learning Disabilities.

You will be responsible for managing a clinical caseload and will liaise with both social and healthcare professionals to meet client need.

You will supervise junior team members and their caseloads by implementing clinical and service pathways and adhering to legislative frameworks.

You will participate in systems and structures, which promote the rights, responsibilities and diversities of people with a learning disability.

The ability to speak Welsh is desirable but not essential for this post.


Main duties of the job

The post holder will promote the health and wellbeing of clients by providing specialist nursing assessments, advice and training to clients, their carer's and other professionals.

The post holder will be involved in the key clinical aspects of providing care, coordinating and reviewing care packages and supervising others.

Job responsibilities

You will have an understanding of the knowledge and skills of the professional activities within a Community Support Team within the field of Learning Disabilities. You will be responsible for managing a clinical caseload, and will liaise with both Social & Healthcare professionals to meet client need.

You will promote the health and wellbeing of clients by providing specialist nursing assessment and advice and training to clients, carers and other professionals.

You will assist the Lead Clinical Nurse in the clinical support and supervision of Band 5 nurses within the nursing team and plan, allocate and evaluate their work.

You will apply specialised knowledge of theory and practice to work with a caseload of complex clients and to challenge appropriateness of care when required. Provide specialist professional advice to commissioning agencies.

You will assist the Lead Clinical Nurse in the management of change to embrace Innovations and developments in health care and their application to practice.

Take an active role maintaining in ensuring a service achieves optimal outcomes for service users.

Person Specification


Qualifications and/or Knowledge

* RNLD Current NMC Registration
* Evidence of Post Registration Education to Degree level or working towards. Tystiolaeth o Addysg l-gofrestru hyd at lefel Gradd neu weithio tuag ato.


Experience

* Evidence of experience working within a multi-disciplinary/multI agency context.
* Experience/knowledge of risk management
* Experience of working with families
* Experience of working with individuals with complex health needs and those with challenging behaviour
* Detailed knowledge of supervision systems and Evidence of clinical leadership
