Advanced Clinical Practitioner (Nurse/Paramedic)

Main duties of the job
* Perform as an autonomous practitioner working independently across our PCN and with other healthcare professionals.
* Manage daily face-to-face, online, and telephone consultations to assess, diagnose, and treat patients with a range of acute, non-acute, and chronic medical conditions, initiating direct referrals as appropriate.
Additional responsibilities
* Work autonomously and be accountable for your professional actions.
* Undertake clinical practice at an advanced level using expert knowledge and skills to deliver holistic primary care.
* Assess undifferentiated patients, take histories, perform examinations, formulate differential diagnoses, and develop management plans with appropriate follow-up.
* Discuss cases with registered GPs and refer to other health professionals when necessary.
* Arrange hospital admissions for suitable cases.
* Conduct telephonetriage, advice calls, and online consultations, including extended access services.
* Manage home and care home visits, including palliative care patients.
* Prescribe medication as a non-medical prescriber when necessary.
* Maintain accurate records using SystmOne/EMIS with templates and READ codes.
* Perform medication reviews and manage medicines effectively.
* Request and interpret tests as needed.
* Handle communications from hospitals and other providers, such as discharge summaries and medication requests.
