*Part-Time Practice Nurse/ Nurse Prescriber*
*Hours:* 12- 15 hours per week (Wed & Friday)
*About the Role*
We are seeking an experienced and skilled Practice Nurse, ideally with an Independent Prescribing qualification, to join our small, rural practice team for two days a week. This is an excellent opportunity for an autonomous practitioner who enjoys the variety and challenge of working across the full spectrum of primary care nursing.
*Key Responsibilities*
* Manage patients with long-term conditions including diabetes, asthma, COPD, hypertension, and cardiovascular disease through structured reviews and proactive care planning
* Provide comprehensive women's health services including contraceptive advice, cervical cytology screening, and management of menopause
* Deliver the childhood immunisation programme in line with national schedules
* Conduct travel health consultations, provide travel risk assessments, and administer travel vaccinations
* Monitor anticoagulation therapy through INR testing and dose adjustments
* Prescribe medications independently within scope of competence and in line with national guidelines and practice protocols
* Support patient self-management through education and collaborative care approaches
* Maintain accurate clinical records and contribute to practice quality improvement initiatives
* Provide health education & lifestyle advice to promote good health
* Wound care and assessment
* Ensure infection prevention and control standards are always upheld
*Essential Requirements*
* Registered Nurse with valid NMC registration
* Cervical cytology training and competence
* Current immunisation and vaccination training
* Experience in long-term condition management
* Strong clinical assessment and decision-making skills
* Excellent communication and interpersonal abilities
* Ability to work independently and prioritise workload effectively
*Desirable*
* Qualification in diabetes care, asthma/respiratory management, or cardiovascular care
* Nurse Independent Prescribing qualification with valid NMC registration
* INR monitoring training and competence
* Experience managing & prescribing in minor illness
* Tissue Viability and Leg Ulcer CAre
* Experience with EMIS clinical IT system
* Quality improvement or audit experience
