Overview
An exciting opportunity has arisen for an experienced long term condition clinical lead nurse to join a busy multidisciplinary team at HMP Deerbolt. The post holder will practice independently and autonomously delivering care and treatment to patients at HMP Deerbolt. The role requires a wide range of knowledge and clinical skills in a Primary care setting. We aim to be flexible in matching your availability with our work patterns and the needs of the service, and therefore we are able to offer part time and flexible working options.
Responsibilities
* Deliver a high standard of care on the treatment of minor illness and minor injury.
* Manage long-term conditions such as Diabetes, COPD, asthma, heart disease, and support cancer screening services.
* Deliver Public Health England health promotion programs such as Immunisations, women’s health, and lifestyle conditions including smoking and obesity.
* Take on management, leadership and development responsibilities for the healthcare team.
* Demonstrate excellent organisational and time management skills to ensure clinics and services are managed efficiently and safely without compromising patient care.
