The Links Medical Practice is a busy, well-established GP practice operating across two sites within the London Borough of Bromley.

We are delighted to offer an excellent opportunity for a highly motivated and enthusiastic individual to join our friendly and supportive team.

This is a permanent, part-time position, offering a flexible shift pattern. Working hours will be between 7:45am and 6:45pm, Monday to Friday, to meet the needs of the service.

The successful applicant will be required to work across both practice sites:

Mottingham Surgery 198 Court Farm Road, London, SE9 4JS

Downham Surgery 27 Brook Lane, Bromley, BR1 4PX


Main duties of the job

We are seeking a proactive, highly organised, and motivated Care Co-ordinator to manage the recall and review process for patients with long-term health conditions. This role is essential in ensuring patients are invited for their routine reviews in a timely manner and that all elements of their care are completed with the appropriate clinicians.

The Care Co-ordinator will also be responsible for supporting the accurate and timely registration of new patients, ensuring that all necessary processes are followed correctly.

Previous experience working within a GP practice environment

A strong understanding of primary care processes and workflows

Experience managing patient recalls, reviews, and clinical administration


About us

The Practice is led by four GP Partners, supported by a Practice Business Manager and an Operations Manager, and a dedicated multidisciplinary team. This includes a further four salaried GPs, three Physician Associates, and six members of the nursing team, alongside approximately fifteen administrative and reception staff.

We also work closely in collaboration with two neighbouring practices as part of a Primary Care Network, enabling shared learning, service development, and improved patient care.

Job responsibilities

The Care Coordinator will provide administrative, organisational, andpatient focused support to help improve care delivery and patient experience.

The post holder will work closely with GPs, nurses, healthcareassistants, and the wider practice team to support patients, particularly thosewith long term conditions or additional support needs. The role involvescoordinating care, assisting patients in navigating healthcare services,supporting clinics, and contributing to the smooth running of the practice.

The Care Coordinator will help ensure patients receive timely,appropriate, and well coordinated care, supporting personalised care planningand continuity of care.

Experience

* Ability to actively listen, empathise with people and provide personalised support in a non-judgemental way.
* Ability to provide a culturally sensitive service supporting people from all backgrounds and communities, respecting lifestyles and diversity.
* Commitment to reducing health inequalities and proactively working to reach people from diverse communities.
* Ability to support people in a way that inspires trust and confidence, motivating others to reach their potential.
* Ability to communicate effectively, both verbally and in writing, with people, their families, carers, partner agencies and stakeholders.
* Ability to identify risk and assess / manage risk when working with individual.
* Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the care co-ordinator role e.g. when there is a mental health need requiring a qualified practitioner.
* Ability to maintain effective working relationships and to promote collaborative practice with all colleagues.
* Ability to demonstrate personal accountability, emotional resilience and work well under pressure.
* Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines.
* Ability to work flexibly and enthusiastically within a team or on own initiative.
* Knowledge of, and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
* Demonstrable commitment to professional and personal development.
* Proficient in MS Office and web -based services.
* Experience of working in Primary Care.
