What you will do: Carry out thorough assessments and coordinate services for individuals wishing to remain in their homes or requiring relocation into care environments. Engage with adults from diverse social and cultural backgrounds, providing specialist advice and support. Offer psychosocial support to service users with life-threatening illnesses and advocate for their and their families' needs. Respond to referrals and manage a varied caseload, ensuring a high standard of practice in line with local policies. Conduct person-centred assessments, evaluating service users needs and adjusting care plans as necessary. Collaborate with a multidisciplinary team to integrate psychosocial aspects into the care of service users. Provide training and mentorship to less experienced social workers and other staff as delegated by the Senior Practitioner.
