Overview
Sherwood Forest Hospitals are seeking a dedicated and proactive Discharge Specialist Nurse to join our team, focusing on discharge coordination. The successful candidate will play a pivotal role in ensuring the timely and effective discharge of patients across the Trust. Working closely with ward-based multi-disciplinary teams and the discharge hub, you will:
* Ensure the provision of specialist nursing advice.
* Develop and implement policies to manage the discharge process.
* Ensure all patients with long lengths of stay over 14 days have clear discharge plans.
* Conduct Reason to Reside checks for all patients.
* Participate in the Length of Stay (LOS) report weekly meetings and other relevant meetings.

Qualifications and Experience
Knowledge Requirements (Essential)

Experience (Essential)
* Experience at a Band 5 or above with specialist skills and knowledge in assessing the needs of patients with complex health and social care needs and extensive experience within medical / care of the elderly nursing.

* Evidence of multidisciplinary working at an advanced level.
Desirable
* Knowledge of the legislation for discharge processes
* In depth knowledge of continuing health and social care policies
* Policy writing and interpretations

* Recent participation in clinical audit and / or research project
