London Borough of Lambeth are looking to expand their current team with a dedicated Hospital Social Worker looking for an opportunity to join a well-structured and organised team. This Hospital Social Worker role will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance.
Hospital Social Worker Responsibilities include:
1. Working with Vulnerable Adults within a hospital setting
2. Report writing and making assessments.
3. Developing care plans
4. Care management
5. Managing complex cases
6. Liaising with other professional bodies
7. Arranging safe and timely discharges for service users
8. Self-directed support / personal budgets
Experience preferred / Minimum Requirements:
9. Post qualifying experience of working as a Hospital Social Worker within the UK Social Care industry.
10. SWE Registration
11. 1 year post qualifying experience within UK Social Care industry
12. Enhanced DBS/CRB (we can obtain for you)
13. Eligibility to work in the UK
