An excellent opportunity is offered for an experienced Community Nurse who holds or is working towards their SPQ in District Nursing to apply for the post of Band 6 District Nurse Caseload holder post.
The successful applicant will hold responsibility for the care and management of a defined clinical caseload of patients within a very busy District Nursing Team within an Integrated Community Network Team.
There are multiple demands in this exciting role due to the challenging nature of providing care for patients outside of the hospital environment.
The post holder will be well supported by an inter‑disciplinary team, which includes social work, occupational therapy, physiotherapy, speech and language therapy, dietetics and pharmacy.
Main duties of the job
The post holder will be responsible for providing support to the Team Leader in the day‑to‑day management of multi‑disciplinary/multi‑agency packages of scheduled and unscheduled care for patients with multiple, complex and palliative care needs.
Assisting the Team Leader in the requirements of the nursing team by utilising effective management and leadership skills.
Responsible for the delegation of clinical nursing activities to appropriately skilled team members, ensuring that an efficient and effective service is delivered to the practice population.
Responsible for prioritising delegated home visits according to the patients' clinical need and geographical location.

Person Specification
Experience
* Working knowledge of safeguarding procedures
* Leadership skills, ability to implement change in response to local/national developments/research
Skills
* Analytical and judgemental skills to enhance the decision‑making process to resolve complex issues/situations
* Understanding and working knowledge of Continuing Health Care, the Nursing Needs Process and completion of these assessments
* Evidence of working within an Integrated Team.
* Experience of working within health and social service systems.
* Welsh language skills in Speaking/Listening at Level 3 or above.
Abilities
* Has or is working towards DN SPQ
* Able to effectively use the Civica scheduling tool
* Illustrates a sound knowledge of effective caseload management
* Understand role in supporting Band 7
* Is committed to own and teams CPD
* Full range of pertinent clinical skills
* Ability to recognise and act accordingly when risk is identified.
* Understand the role of the MDT and make pertinent referrals.
* Understanding of the DN interim principles and how these relate to practice.
* Ability to travel between work bases in an independent and timely manner.
* Able to evidence conflict management skills.
