What We're Working On

* Setting up a dedicated clinic space within a local remand prison
* Improving the transition pathway from secure care to community services
* Embedding forensic specialists into each Community Mental Health Team (CMHT)
* Providing consultation and formulation support to staff and services
* Creating a co‑production forum for collaborative service development
* Offering targeted risk reduction interventions in the community
* Delivering comprehensive forensic training for all CWPT staff
* Leading research on community forensic services to shape future care


Main Duties of the Job

As part of a multidisciplinary team you will support service users’ transition into the community by:

* Providing Care Act and funding liaison to ensure a safe and effective transition
* Working closely with multiple service providers and other agencies
* Voice your professional experience to support patients and other services that may need your advice or support
* Participating in team meetings and responding to queries as required
* Receiving supervision and appraisal support to develop your skills and reach your potential

Job Description

In this role you will be instrumental in supporting service users' transition into the community. You will liaise across services, provide Care Act knowledge, secure safe and effective transitions, and deliver specialist forensic support to multidisciplinary teams. Your expertise will inform patient care plans and service design, and you will contribute to research and development objectives.


Person Specification


Essential Qualifications

* Relevant professional qualification (e.g., RMN, DipCOT)
* Current unrestricted registration with HCPC/NMC
* Specialist knowledge across a range of procedures and theory
* Professional knowledge acquired through degree/diploma
* Relevant training and experience in specialist short courses
* Evidence of continuing professional development
* Full driving licence and access to a motor vehicle for business use


Essential Experience

* Substantial experience in a community mental health care setting
* Developed specialist knowledge gained within a similar environment over a considerable period of time
* Significant experience of undertaking assessment, formulation, risk assessment and positive risk management
* Experience of safeguarding and investigations
* Proven experience of people management


Essential Skills

* Excellent written communication skills
* Strong collaboration and relationship‑building abilities
* Knowledge of relevant legislation and forensic practice standards
