We are looking to recruit a paramedic with 5 years of experienceto work as part of a multi-disciplinary team. The role is community based, but will include some work at our three sites in Hedge End, Hamble and Lowford.

Paramedics areemployed by the Primary Care Network to support population health managementthrough on-the-day demand and access with home visits, treatment of minorailments and injuries and other support relevant to General Practice.


Main duties of the job

To be part of a multi-skilled primary care team providing patientcentred medical services based on patient needs with a focus on continuity ofcare, including,

Working alongside the team within the primarycare network to provide a first point of contact for patients presenting withmedical problems at home, making use of skills in history taking, physicalexamination, problem solving and clinical decision-making, to establish adiagnosis and patient centred management plan.

It will also help safeguard GP services formany years to come.

The merging of two established and resilient practicesprovide the opportunity to create a new organisation on a long term sustainablefooting able to capitalise for patients the strengths and opportunities forimprovement that come with scale whilst still supporting the fundamentals ofgeneral practice so valued by patients and the GP partners.


Job responsibilities

Lead certain community services i.e. monitoringblood pressure and diabetes risk for elderly patients.

Perform urgent and same-day home visits.

This role will work as part of the PCN MDT,working collaboratively with primary care network staff to promote integratedand seamless pathways of care.Instigate necessary investigation/non-invasivediagnostic tests/investigations and interpret findings/reports.

Refer patients to an alternative care setting ortreat and discharge as appropriate.

Contribute to the locality practices achievingits quality and quantitative targets to sustain the high standards of patientcare and service delivery.

Help to develop and set up new patient servicesand participate in initiatives to improve existing patient services.


Person Specification


Experience

* -In total 5 years previous experience in a paramedic role.
* -The post holder will be expected to work with a high degree of independence and manage their own workload according to priorities.
* -To establish effective networks with internal and external stakeholders.
* -Demonstrable patient assessment skills.
* -Knowledge of general practice related medical conditions, treatments and pharmacology.
* -Knowledge of maintaining confidentiality and implementing data protection legislation requirements.
* -Knowledge of safeguarding principles.
* -Ability to work as a team member and autonomously within own scope of practice, understanding when to refer to GPs.
* -UK Driving Licence and own vehicle.


Qualifications

* Health & Care Professionals Council registration.
* Registered paramedic with 5 years post registration experience.
* Ability to use EMIS medical system


Experience

* To establish effective networks with internal and external stakeholders.
* Team player.
* Following agreed clinical protocols with referral to senior healthcare professionals or GPs as appropriate.
* Able to document consultations according to Health & Care Professionals Council HCPC guidelines.
* Self-motivated individual.
* Promote and protect the interests of service users and carers
* Work within the limits of their knowledge and skills
* Report concerns about safety
* Respect confidentiality
