Position Overview
An exciting opportunity for a band 6 nurse to join the SNOP team (Specialist nurses for older persons). The SNOP nurses are based throughout the trust, including Acute care, DME, ED and the frailty unit. The successful candidate will holistically assess older patients at the front door, aiming to reduce risk of deconditioning while awaiting diagnostic tests and decide on discharge or admission.
Responsibilities
* Assess, plan, implement, and evaluate care for older frail patients, offering advice and support to ward staff, patients, and their families.
* Provide expert clinical advice and necessary skills for patient assessment such as catheter insertion, cannulation, ECG.
* Participate in the education and development of other staff members.
* Work collaboratively with clinical colleagues to develop services and improve quality of care.
* Ensure the management plan following CGA is commenced for all relevant patients and aim to complete within 1 hour of admission.
Qualifications
Minimum of 2 years qualified or experience in frailty care. Will provide specialist training and support from the MDT team.
