The maternity team at CDDFT are searching for a committed,dynamic and enthusiastic midwife to join our Durham community midwifery teambased in Chester le Street.

We want you to be here, helping to shape our vision for anoutstanding service that gives women, families, our community and our midwivesan extraordinary experience. You will join a warm and supportive team ofcommunity midwives who truly care about the whole maternity experience.

We welcome applications from band 5 Midwives who arecurrently due to complete their preceptorship to band 6.

We are seeking a community midwife who is enthusiastic andcommitted to working in collaboration with women & birthing people,families and the multi-disciplinary team, supporting personalised choice andpromoting exemplary care in line with national transformation ambitions and ourtrust values.

Main duties of the job

We are looking for a passionate, committed band 6 midwife toprovide maternity care within our community midwifery team based at Chester leStreet. You will be responsible for the planning, implementation and evaluationof individualised care plans which ensure delivery of holistic, research basedcare, prioritising communication, informed consent and relationship building.CDDFT community midwives are passionate about facilitating antenatal andpostnatal continuity of care and you will be supported as an autonomouspractitioner to plan your diary around your caseload, ensuring that women andfamilies are at the centre of everything we do.

Our team at Durham provide care to women andfamilies across a varied urban and rural setting. Our community hospital works closely with ourmaternity units at the University Hospital North Durham and at DarlingtonMemorial.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

We are looking for an enthusiastic and dedicated band 6Midwife who is keen to join and help further develop our community midwiferyteam based at Chester le Street Community Hospital. We have so many newdevelopment opportunities on the horizon at CDDFT it is a very exciting time tobe a part of our Maternity family!

Our team at Durham and Chester le Street provides care towomen and families across a varied urban and rural area. We have a very diversecase load, including vulnerable and isolated communities, providingopportunites to make a real difference to the families in your care. We have awell established and well led team of midwives within the team, who you willfind very welcoming and supportive. The team are assisted by the widermaternity team, including a dedicated Community Matron and Matron for PublicHealth and Inequalities.

We are very keen to encourage community midwives with aninterest in providing care across the pregnancy and postnatal journey to applyto work with us to deliver outstanding community based Antenatal and PostnatalCare to the mothers and families in this area. We are also commited toprovision of our home birth service and would particularly welcome communitymidwives with an interest in further developing this important area of care.The focus of our dedicated community teams is patient centred andindividualised care, providing support and continuity throughout the antenataland postnatal period and promoting choice to families from across our diversecommunities.

We are looking for an individual with a passion for healthpromotion, as the role of community midwife at our trust includes workingwithin the UNICEF framework for Infant Feeding, and close working relationshipswith our specialist antenatal and newborn screening Team to deliver allelements of the National Screening programme.

You will be very well supported with a bespoke induction andtraining package by members of our specialist Practice Development Team whowill ensure your support as you transition to your new role both professionallyand personally. The successful candidate will be supported and encouraged toseek educational opportunities to further their Continuing ProfessionalDevelopment and career progression. We are a trust which actively supports andencourages lifelong learning and development and we are very keen to supportyou to achieve your full potential with us. In turn, you will have theopportunity to assist in supporting the development of our preceptorship,Internationally Educated and student midwives.

We strongly encourage a culture of life-long learning anddevelopment here at CDDFT and you will be encouraged and supported to continueto develop and thrive in your role as a community midwife. We offer a fullrange of CPD funded opportunities, including NIPE and NLS training and arepassionate about promoting leadership and development opportunities to ourmidwives.

We are very proud of our team and we are certain there is arole you will enjoy within our community midwifery service at CDDFT.

All of our teams work under the care of a dedicated teamleader in each team, overseen by ourCommunity Matron and the Matron for Public Health and HealthInequalities. Each team also has access to admin support and band 3 MaternitySupport Workers who offering close support to each team. You will also be ableto access personalised pastoral care via our specialist workforce experienceteams, including our retention lead midwife and the PMA service.

Qualifications

* Registered Midwife with current UK registration and NMC PIN
* Evidence of Ongoing Professional Development
* Recognised teaching /assessing qualification
* Trained NIPE practitioner
