Job Summary
This is an exciting opportunity to join a growing team of Clinical Pharmacists and Pharmacy Technicians. Due to the expansion of the team and services, Primary Healthcare Darlington is looking for a highly motivated and aspirational clinical pharmacist to join our established team.
The role supports the delivery of the PCN Contract DES specifications and aims to lead improvements in safe, evidence-based practice to enhance patient care across Darlington and surrounding communities.

This position is funded through the Additional Roles Reimbursement Scheme (ARRS) and involves working to support GP practices within the PCN.
Main duties of the job
The Clinical Pharmacist will support the Lead Pharmacist & other Pharmacy colleagues in medicines optimisation, managing hospital discharge medication changes, secondary care medicines, and practice queries. Responsibilities include conducting telephone consultations, medication reconciliation, chronic disease management, repeat prescription management, and structured medication reviews, including patients with polypharmacy.

Job Responsibilities
Contribute to effective transfer of care processes, support colleagues and patients with medication queries, undertake clinical reviews, provide expertise in medicines use, address health inequalities, lead medicines optimisation, support development of new services, ensure CQC compliance, develop professional relationships, and support pharmacy teams.
Person Specification
Essential: Degree in Pharmacy, GPhC registration, completion of CPPE pathway or equivalent.
Desirable: Independent prescribing qualification.
