An exciting opportunity has arisen for a suitably qualified and motivated person to join the Older Peoples Mental Health Liaison Team as a Mental Health Practitioner. This award winning team is based at The Meadows within the borough of Stockport. The service provides a resource into a variety of settings to support a range of disciplines from health, social care and the third sector in order to meet the mental health needs of our service users.
The post holder will work as part of the multi-disciplinary team and undertake allocated duties within the Older Peoples Liaison team whilst also working alongside adult social care in our neighbourhood teams. You will provide specialist mental health nursing advice and assessment skills in conjunction with community services staff in order to develop appropriate interventions and treatment plans for older people with mental health problems. This post is full time.
