Overview
An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Garioch cluster. The role is a diverse and autonomous clinical role that delivers positive outcomes for patients by providing enhanced clinical assessment and treatment within the community setting. You will support medical staff and other healthcare professionals to deliver optimal service delivery, work in collaboration with independent practices to provide a high level of care throughout clusters, undertake acute on‑day triage and perform home visits when required.
Key Responsibilities
* Provide enhanced clinical assessment and treatment to patients in GP surgeries, community hospitals, and patient homes.
* Support medical staff and healthcare professionals to deliver optimum service delivery in the community.
* Collaborate with independent practice partners to ensure high‑quality care across clusters.
* Undertake acute on‑day triage and home visits as required.
* Manage patients in an MDT team, recognising and initiating clinical management of deteriorating patients.
* Proactively expedite the patient journey and develop the ACP role within Aberdeenshire.
Qualifications and Experience
* Advanced Nurse Practitioner or Advanced Clinical Practitioner qualification (or equivalent).
* Sound understanding of conditions that may require hospital admission or virtual community ward placement.
* Experience in managing patients at home as part of an MDT team.
* Proactive approach to expediting patient journey and developing ACP role.
* Mandatory completion of the Advanced Care Academy framework portfolio within an agreed timeframe.
Working Hours & Conditions
* Flexible working hours to meet service needs; part‑time hours may be considered.
* A valid driving licence and access to a vehicle are essential.
* Willingness to travel within the Garioch cluster is required.
