Specialist Community Public Health Nurse Triage (SCPHN)

Job overview

The post holder will be a senior member of staff working as a triage practitioner within the integrated 0 to 19 Partnership. They will provide the expertise, knowledge, resource and support to the team managing enquires from pregnant women, families, carers, children and young people in Torbay.

The post holder will provide a comprehensive triage service (for the public and professionals), access and update the Electronic Health record to ensure continuity of care and to make appropriate recommendations and interventions.

They will work with the 0 to 19 Partnership workforces to ensure pathways are followed in supporting children, young people, families and carers.

The service includes the identification of vulnerable children, including those whose welfare and safety may be at risk, working within child protection procedures and in collaboration with other agencies.

The post holder will have the opportunity as part of a team to shape the ongoing development of the role within the Universal Team allocation process and be the Advice line first point of contact for families contacting the Admin Hub.

The post is office based.

Person specification


Qualifications and training

* Active Registration on Part 1 of NMC Register.
* Evidence of CPD in last 2 years.
* Educated at Degree Level
* V150 Nurse Prescriber Qualification
* International Computer Driving Licence


Knowledge and experience

* Ability to manage conflict
* Knowledge of 0 to 19 Torbay contract and Key Performance Indicators
* Ability to work as part of a team
* The ability to manage constant change
* Ability to be reflective on situations and show learning
* Demonstrate the ability to remain focused on service delivery whilst dealing with an unpredictable, diverse and challenging workload
* Demonstrate research skills in current role
* Coaching skills
