Community Nurse Case Manager – Complex Care Location: Chesterfield & surrounding areas Salary: Circa £40,000 (negotiable depending on experience) Full-time | Monday–Friday 9am–5pm (flexibility required) We are working with a well-established Complex Care provider who has recently acquired a new service in Chesterfield and is now looking to grow and develop their Complex Care division in the area. As part of this exciting expansion, they are seeking a dedicated and passionate RGN-qualified Nurse Case Manager to play a key role in shaping and developing services locally. At present, the service delivers around 3,000 hours of homecare, and this role will be mainly office-based initially, supporting service development, clinical governance, and workforce readiness while new complex care packages are grown and established in the community. The Role As a Community Nurse Case Manager, you will manage a caseload of clients with complex needs, supporting them to live safely and independently at home and within their wider communities.
