An exciting opportunity has arisen for a Clinical Pharmacist to be part of our growing team supporting the Weymouth & Portland Primary Care Network (PCN). This role is funded under the Additional Roles & Responsibilities Scheme. It is an interesting and varied role, working directly with other PCN pharmacists, pharmacy technicians, and GP leads to develop the structured medication review (SMR) and medicines optimisation services within and across the five GP Practices of our PCN.

Due to the evolving nature of this role, a draft JD has been provided.

Most work will be undertaken from a central location within Weymouth, but travel between network practices will also be necessary. The successful candidate will have a role in managing patients' long-term conditions; working to assess and treat conditions, as well as promote self-care.

The candidate will have excellent written and verbal communication skills, experience of influencing others, good clinical knowledge, and an understanding of prescribing data.


Main duties of the job

The successful applicant will work as part of a multidisciplinary team, working closely with all primary care colleagues with direct input and support from general practitioners, to deliver Structured Medication Reviews (SMRs), reauthorisation of repeat prescriptions, management of medicines on transfer of care, and systems for safer prescribing, among other key roles as outlined in the Network DES minimum role requirements.

Job responsibilities

The post holder will work within their clinical competencies as part of a multidisciplinary team to provide expertise in clinical medicines management, deliver structured medication reviews, manage long-term conditions, oversee medicines on transfer of care, implement systems for safer prescribing, manage repeat prescription authorisations, handle acute prescription requests, and address public health and social care needs of patients in the GP practice(s) within the PCN.

The post holder will contribute to quality improvement and clinical audit activities as well as support the delivery of the Integrated Care System's Prescribing Quality Scheme.

This role is pivotal to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver an excellent service within general practice.

The post holder will be supported to develop their role to become a non-medical independent prescriber after completing the PCPEP, if not already qualified.

Person Specification


Experience

* Evidence of involvement in developing care programmes, protocols, and audits
* Experience working in a multidisciplinary team
* Experience of education and training in practice
* An understanding of primary care prescribing, rational prescribing concepts, and strategies for improvement
* Experience working in general practice & PCN setting
* Understanding of the FP10 data management system (ePFIP)
* Experience coordinating complex activities with professionals and agencies


Knowledge and Skills

* Vocational master's degree in Pharmacy (4 years or equivalent), plus one-year pre-registration training
* Registered with the General Pharmaceutical Council
* Willing to enroll and complete the Primary Care Pharmacy Education Pathway
* Working towards or holding an independent prescribing qualification
* In-depth therapeutic and clinical knowledge with evidence-based healthcare principles
* Understanding of GPs, general practices, and PCNs
* Awareness of common acute and long-term conditions in general practice
* Ability to plan, manage, monitor, advise, and review medicine optimization issues in long-term conditions
* Postgraduate clinical pharmacy diploma or equivalent experience, including completion of the clinical pharmacy pathway
* Knowledge of NHS primary and secondary care working, including prescribing management across interfaces
* Member of or working towards Faculty membership of the Royal Pharmaceutical Society
* Minimum of 5 years post-qualification experience

£26.50 an hour starting rate, dependent on experience
