You will be responsible for providing social work services to individuals with long-term complex conditions and disabilities. This includes early and flexible intervention with a focus on prevention and self-management to help individuals maintain their independence. You will also determine eligibility for the provision of services and equipment, promote user empowerment, and provide information and guidance to connect individuals with appropriate services.
