About The Reablement Service
Our short-term, therapist-led service helps adults to regain skills, confidence and independence following illness, injury or a change in ability. Youll work with customers, families, Health colleagues and community partners to design creative, goal-focused reablement plans that reduce the need for long-term support.
About The Role
As an Experienced Occupational Therapist, you will:
* Manage a varied and busy caseload, including complex and challenging cases.
* Complete strength-based OT assessments and design innovative reablement plans tailored to individual goals.
* Carry out complex risk assessments, including moving and handling assessments, and provide clear professional recommendations.
* Assess for and recommend minor and major home adaptations in line with the Care Act and housing legislation.
* Work closely with customers, families, carers and multidisciplinary teams to monitor progress and adjust plans as needed.
* Provide professional support and supervision to Occupational Therapists, Customer Coordinators and wider team members.
* Contribute to service development and promote evidence-informed, high-quality practice.
* Ensure all practice aligns with safeguarding responsibilities and council policies.
About You
You will be a proactive, confident practitioner who can work independently and collaboratively within a supportive team.
Essential Criteria Include
* Professional qualification in Occupational Therapy (diploma or degree).
* Registration with HCPC as an Occupational Therapist.
* Minimum of 2 years post-qualification experience, including work with adults.
* Strong knowledge of Health, Housing and Social Care legislation.
* Significant experience delivering adaptations, moving and handling assessments and reablement interventions.
* Ability to prioritise, problem-solve and manage complex situations.
* Excellent communication, report-writing and interpersonal skills.
* Ability to supervise and support staff effectively.
Desirable Criteria Include
* Experience in a social care, reablement or intermediate care setting.
* Experience supervising students or staff.
* Moving and handling training experience.
Other Role Requirements
* Ability to travel regularly to customers homes and other locations.
* Occasional out-of-hours work (less than 10% of contracted hours).
* Enhanced DBS check required.
* Some roles may require a valid driving licence depending on classification.
