We are a forward thinking, single practice PCN, based in Plympton andIvybridge (Devon). We have a list size of 43,000 patients. Main duties of the job
Key ResponsibilitiesTo support the practice teamto ensure the delivery of safe, effective and efficient systems for repeatprescribing, medicines optimisation, reducing medicines waste and maximisingpatient outcomes.
The Pharmacy Technician willresolve prescription related issues and encourage better pharmaceuticalunderstanding with patients and staff throughout the primary healthcare teamincluding GPs, prescription clerks, patient advisors, nurses, and externalcommunity organisations.
Working as part of amultidisciplinary team, your role will support improvement, quality and safetyof all aspects relating to the prescribing of medicines within thepractice.
The post holder will be partof the wider Beacon Medical Group team and will work effectively alongsideother teams and clinicians within the Group in a professional manner to nurturea spirit of collaborative team working. Job responsibilities
Toundertake medicines reconciliation in accordance with practice protocols and toupdate patients medical records / prescribing accordingly.
Carry outmedication reviews whilst in patient records e.g. dose optimisation andmedicines synchronisation
Supportthe prescription clerks by helping with queries relating to prescriptionrequests and look for ways to improve the efficiency and safety of theprescription issuing process, ensuring queries are resolved and scripts issuesin line with the timeframe in the practice procedure.
Interfacewith community and hospital pharmacy colleagues on medicines related issues.
Implementand support use of monitored dosage systems and support the district nursingteam with medicine optimisation for housebound patients
Implementand support electronic prescribing and repeat dispensing in appropriatepatients including working with the community pharmacy staff to identifyappropriate patients.
Reviewout-patient letters which include changes to drug treatment and make changes topatients repeat medication list as appropriate (or query prescribing if not inaccordance with Formulary). Liaising with patients and community pharmacyregarding changes.
Reviewout-patient discharge recommendations and issue prescriptions as requested
To supportthe achievement of the practices prescribing Quality and Outcomes Frameworktargets.
Work with the Lead Clinical Pharmacist, Pharmacists and Clinicians inaccordance with agreed practice prescribing
Workingwith the Clinical Pharmacist and Prescribing Lead GP to contribute to thereview and development of repeat prescribing protocols / Standard OperatingProcedures to improve the efficiency and effectiveness of repeat prescribingsystems.
To helpdrive quality improvements in areas of own workload that will improvePopulation Health management and targets set by the Improvement InvestmentFund.
To provideadvice to GPs, staff and patients in changes to prescribing to support theimprovement of prescribing safety, quality and cost effectiveness.
Toparticipate in medication audits and ensure findings are recording and sharedwith the wider team.
To assistin the appropriate monitoring and management of prescribing the budgets and toliaise with the medicines management team at the ICB.
To prepareevidence based resources and information to support the implementation ofrational cost-effective prescribing.
Providesupervision for the wider team to ensure staff are carrying out their dutiesappropriately; in accordance with local policies and SOPs.
To supportthe team to ensure dispensing standards are met and achieved and relevantaudits are completed to meet annual DSQS requirements.
To supportthe Clinical Pharmacists to provide medication reviews, patient informationleaflets, medicine awareness; and to undertake community visits asappropriate.
To take aproactive approach to the safe handling of prescriptions, including assessmentof risk and making recommendations for improvement.
To respondto medication queries from patients and staff in a professional mannerreferring to the appropriate GP in accordance with practice protocols.
To adviseon the sourcing and safe management of medicines as appropriate.
To usehighly developed knowledge and skills to provide high standards of patientcentred care.
To workwith the Care Home Pharmacist and offer Pharmacy Technician support whereappropriate. This support could include reviewing and highlighting patients tothe care home pharmacist in relation to structured medication reviews andsupport for polypharmacy, deprescribing, quality improvement, appropriate useof nutritional supplements and improving medicines related processes in carehomes
Undertakingpatient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients
To provide medication review services to patients via clinics inpractices, domiciliary visits and in residential and nursing homes and todeliver pharmaceutical care plans that maximise cost-effective prescribing andimprove the quality of patient care.
Governance
The post holder will assist the Lead Clinical Pharmacist with data analysis of prescribing behaviourin line with NICE guidance and local clinical commissioning directives.
Tosupport the safe and effective repeat prescribing of high-risk medicinesensuring that regular monitoring as per local/shared care guidance is takingplace.
To document interventions made to patients medications in their health carerecords to ensure there is a clear audit trail and changes are clearlyaccounted for
Awarenessof, and compliance with, all relevant local/clinical policies and guidelines

Offeringsupport and training to student placements with both Pharmacy Team trainees andthe wider team. Pharmacy Technicians can also support new staff to understand prescribing processes within the clinicalsystem and policies across the wider group in relation to safe and effectiveuse of medicines
Tosuggest and offer training for the dispensing team to ensure correct standardsare met. This for example could be through audit/quality improvement projectsand sharing best practice from national or local bodies

Person Specification
Experience
* Understanding of prescribing and Medicines Management issues
* Knowledge of the principles of medicines optimisation
* Understanding of working effectively within multidisciplinary groups
* Evidence of great Customer Service skills to be able to deal with patients conversations effectively both face to face and via the phone
* Actively identifies areas of improvement
* Knowledge and understanding of pharmacy law and ethics and current legislation
* Understanding of current national policies, in particular those that impact on medicines management services
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
* Minimum 2-3 years relevant experience post NVQ3
* SystmOne clinical system
* At least 2 years experience working as a qualified Pharmacy Technician within a Primary Care/GP setting
* Evidence can work on own initiative and be able to prioritise workload effectively
* Experience of Audit work and reporting
