The aim of the Patient Flow Co-ordinator is to provide support to the Shift Leader in dealing with administrative tasks. The concept is designed to free up valuable nursing and medical time to avoid delays and improve the patient experience.
The Patient Flow Co-ordinator will be expected to work as part of the A & E team, under the direction of the Shift Leader, to ensure the smooth flow of patients through the department. It is expected that by co-ordinating administrative tasks, and liaising with other departments, the post holder will contribute to the efficient and effective running of the A & E department resulting in a significant reduction in patient journey times.
Main duties of the job
Main duties and responsibilities of post holder
1. Providing administrative support to the shift leader to support the smooth flow of patients through the A & E department including having knowledge of all patients that are in the department at any given time and the care plans for these patients.
·Provides clerical support for the Accident and Emergency Department.
·Develops and implements systems and procedures to improve the efficiency and effectiveness of the Accident and Emergency Department, in order that the Department can provide the necessary information required by the Trust and other external organisations.
2. Keeping the A & E computer system (Symphony) up-to-date at all times
3. Highlighting to the shift leader those patients that could potentially breach the 4-hour wait target and then also inform site coordinator and modern matron.
4. Keeping a constant check on the return and review of blood results and following up the return and review of X-Rays.
5. Ensure time of reviews are documented and that specialities attend
6. Act as the main point of contact for the portering service and ensure the porters are informed as soon as possible when patients are ready to transfer.
7. Liaise with the staff on the Wards with regard to bed requests and discuss potential bed breaches with the Bed Manager.
8. Book transport for patients if required and inform the Discharge Lounge of patients waiting for transport.
