Complex Care Coordinator Rochdale / Tameside / BurnleyCompetitive Salary (DOE)Full Time | PermanentJoin a Growing Nurse-Led Complex Care Provider At Complex Care Professionals Ltd, we're not your typical domiciliary care company.We are a nurse-led organisation specialising in complex care, continuing healthcare, end-of-life care and fast-track support, helping people with complex health needs remain safely and independently in their own homes.Working closely with Integrated Care Boards (ICBs), healthcare professionals and families, we deliver high-quality, person-centred care across the North West.Due to continued growth, we are looking for an ambitious and organised Complex Care Coordinator to join our team.If you're passionate about delivering outstanding care, thrive in a fast-paced environment and enjoy building strong relationships with both clients and care teams, we'd love to hear from you.The Role As a Complex Care Coordinator, you will play a vital role in ensuring our clients receive safe, effective and responsive care.You will be responsible for coordinating care packages, supporting care staff and maintaining excellent communication with clients, families and healthcare professionals.No two days are the same, and you'll be at the heart of our operation, helping us deliver exceptional care while supporting the continued growth of the business.Key Responsibilities Coordinate new and existing care packages from referral through to service deliveryManage and optimise staff rotas, ensuring safe and effective staffing levelsRespond to staff absence, emergencies and changing client needsRecruit, onboard and support care staff throughout their employment journeyLiaise with clients, families, healthcare professionals, case managers and commissionersSupport service quality, compliance and CQC standardsConduct staff supervisions, spot checks and performance reviewsBuild and maintain positive client relationshipsSupport package mobilisation and service growthParticipate in the on-call rota as requiredAbout You We're looking for someone who is:Organised, proactive and solutions-focusedExperienced within domiciliary care coordination or a similar care management roleA confident communicator with excellent interpersonal skillsAble to manage multiple priorities in a fast-paced environmentPassionate about delivering outstanding person-centred careComfortable working independently and as part of a teamExperienced using electronic rostering systems (Access Planner experience desirable)In possession of a full UK driving licenceWhy Join Complex Care Professionals?
