As part of the rapid expansion of the General Nursing Assistant (GNA) service, within the Integrated Placement of Care Hub, to support an increasing number of patients in the community, additional staffing is required to ensure that the team are supported, and the service can successfully maintain flow through the care provision. The GNA Care coordinator will assist in the identification of health needs and participate in the delivery and evaluation of care interventions developed to meet those needs under the leadership of a senior registered Nurse or Practitioner. They will provide oversight of the care, aim to reduce care packages as able, support nursing and therapy rehabilitation and care plans and generally support junior GNAs. Supervision may be direct/indirect depending upon competency/complexity of work. They will provide senior cover and support to GNAs on duty including out of hours where they will be responsible for the Buddy procedure in line with the Lone Working Policy. A significant part of the role also involves supporting end of life patients in their own homes, providing a dignified and quality approach to care for the patients and their families at a challenging time. This will involve reviewing end of life patients and working alongside Palliative Care in Partnership IPOCH Nurses and Practitioners, as well as the wider care community teams, including district nurses. The GNA Care coordinator will continue to develop and undertake a range of delegated tasks and will report to a Registered Nurse or Practitioner. The GNA Care coordinator will be expected to provide general/specific care as required for our patient group of both rehabilitative and end of life patients and will be proficient and competent to work across professional disciplines, referring clients for assessment and treatment or to other agencies as required. The GNA Care coordinator will be an essential member of the Integrated Placement of Care Hub in assisting to provide the urgent and planned care to patients in their own homes and care homes preventing avoidable hospital admissions and facilitating earlier discharges into the community. The post holder will work with and support the clinicians by working with the patients to achieve their goals and will be responsible for an agreed specific caseload. The post holder will support and teach practical skills to junior General Nursing Assistants, students, patients and their relatives.
