Job summary
The post holder will be a qualified pharmacist holding an independent prescribing qualification, who acts within their professional boundaries as part of a multidisciplinary team.
Main duties of the job
The post holder will act within their professional boundaries, supporting and working alongside the clinical team,as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to clinicians with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the practice.

We practice from a new Health Centre in a pleasant large village between Mold and Wrexham, 10 miles from Chester and commuting distance to Liverpool and Manchester. Our list of patients is varied, from farmers to factory workers, commuters to stately home owners.

Qualifications
Essential
1. Completion of a degree in pharmacy and registration with the General Pharmaceutical Council;
2. Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this;
3. Qualified independent prescriber, or working towards this;
4. Member of the Royal Pharmaceutical Society (RPS).
Desirable
5. Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice;
6. Ability to demonstrate general pharmaceutical skills and knowledge in core areas;
7. Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions;
8. Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals;
9. Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate;
10. Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct;
11. Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance;
12. Non-judgemental attitude and ability to be flexible and adaptable;
13. Self-motivated with excellent communication skills;
14. Understand the need for patient confidentiality;
15. Knowledge of local services.
Experience
Essential
16. In depth therapeutic and clinical knowledge and understanding of the principles of evidence--based healthcare.
17. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
18. Excellent interpersonal, influencing and negotiating skills
19. Demonstrate the ability to communicate complex and sensitive information in an understandable form to patients
