Overview
You will work within a multidisciplinary team across community, primary care, acute neuro rehabilitation and stroke early supported discharge units, as well as other external agencies. The role is based primarily at Hinchingbrooke Hospital, Huntingdon, with occasional coverage across the South of the county. You will support Neuro Rehabilitation as in‑reach to inpatient beds and in patients’ homes, helping them to overcome physical, cognitive and psychosocial challenges.
Key Responsibilities
* Work autonomously in the community setting under the direction of a registered practitioner within the integrated multi‑professional ESD and neuro rehab team.
* Support patients on the ESD pathway, collaborating closely with the ward multidisciplinary team and within the ESD/CNRS.
* Assess patients’ physical and functional needs, interpret, plan and implement therapy interventions for patients with stroke and neurological problems.
* Use clinical judgement to evaluate patient progress, recording objective and subjective measurements.
* Manage own caseload of patients with complex needs, planning and prioritising workload.
* Undertake duties delegated and supervised by a registered professional, following prescribed packages of care.
* Keep patients at the centre of care, ensuring patient and carer participation in decision making.
Requirements
* Full UK driving licence and access to a personal car for independent travel across the county.
* Ability to travel independently across the work locality and meet strict time deadlines.
* Flexible and adaptable to support service delivery across the organisation.
