Your role:
This is a fantastic opportunity for someone passionate about supporting people through their hospital discharge and onward journey. This dynamic role is an addition to an established Administration and Patient Flow function within the Home First, Independence Social Care Team who work closely with residents, families and multi-disciplinary teams utilising a holistic and asset-based approach to help residents discharge safely and timely. The role requires someone with creativity, initiative, and tenacity.
About you:
You will be highly motivated and enthusiastic with experience of working in a fast-paced environment, meeting a variety of demands whilst maintaining quality and accuracy. You will have a good local knowledge of Community and Voluntary services, including Care Homes. You will be service user focused and keen to make a positive difference to the individuals and team you work with. You will be a great communicator who is used to dealing with a range of people at all levels both internally and externally. You will have strong I.T. and administration skills to support the wider functionality of the team.
