Post: Team Leader (Central Referral Team)
Reports to: Services Manager
Job Purpose
The Team Leader will lead a centralised referral and assessment team responsible for managing all incoming referrals into GAMH Service Centres. A key focus of the role is to develop and maintain efficient and professional working relationships with key external referrers including Primary Care Mental Health Services, GPs, and Social Care Connect. Working in close partnership with Project Leaders of GAMH Service Centres, the Team Leader will ensure that referrals are appropriately screened, assessed, and allocated to the most suitable service team. The team will also support referred individuals by providing high-quality signposting, relevant information, and undertake short-term pieces of work when longer-term engagement is not required.
Key Responsibilities
Referral & Assessment Coordination
• Manage the day-to-day operations of the central referral and assessment function.
• Ensure that all referrals are screened for suitability including risk assessment in line with service criteria.
• Oversee timely and person-centered assessments for each referral received.
• Allocate referrals to the appropriate GAMH Service Centre based on assessed need.
Stakeholder Engagement
• Build and maintain positive, collaborative relationships with key referrers such as:
• Primary Care Mental Health Services • General Practitioners (GPs)
• Social Care Connect
• Represent GAMH in external meetings to promote service understanding and access.
Team Leadership
• Line manage a team of referral workers, providing regular supervision, support and performance reviews.
• Foster a supportive and accountable team culture that reflects GAMH’s values.
• Ensure all team members receive appropriate training and professional development.
Short-Term Interventions & Signposting
• Support the team in delivering brief interventions and short-term support when appropriate. • Oversee signposting to other services or resources where GAMH is not the best fit.
• Ensure individuals receive compassionate and respectful support throughout their time at GAMH
Quality Assurance & Reporting
• Monitor referral processes and team performance to ensure service quality and efficiency.
• Maintain accurate and confidential service records in line with GDPR and safeguarding practices.
• Ensure that the team are aware of the legal and work settings policies, procedures and practices required for health, safety and security relating to their work.
• Contribute to internal reports, audits, and service evaluations as required.
