Job Title: Occupational Therapist - Mental Health Discharge Team

We are seeking an enthusiastic, motivated, and forward-thinking Occupational Therapist to join our Mental Health Hospital Discharge Countywide team on a fixed-term contract for 12 months. You will work with working-age adults (18-70) and older adults (70+), supported by a caring team dedicated to both your well-being and that of our community.
Your role will involve supporting effective and timely hospital discharges across East Sussex, covering all acute hospitals for patients admitted for mental health treatment. This is a new initiative, where you will collaborate remotely with Sussex Partnership Foundation Trust (SPFT) and the voluntary sector to identify safe, creative discharge options centered around the individual.
You will work closely with other stakeholders to develop optimal discharge plans.
Candidate Requirements:
* Qualified Occupational Therapist registered with the relevant professional regulatory body
* Ability to build positive relationships with clients, families, and professionals
* Self-motivated with a commitment to training and personal development
* Willingness and ability to meet travel requirements of the role

This position requires an Enhanced DBS check or an equivalent for Regulated Activity. Evidence of an overseas police check is needed if you have lived or worked abroad.
