We are currently seeking an experienced Social Worker for a locum Advanced Practitioner role within a Continuing Healthcare (CHC) team on the Wirral. This is an excellent opportunity to take on a leadership-focused position while making a meaningful impact in a flexible, temporary assignment.

Undertake Continuing Healthcare (CHC) assessments and reviews
Apply the Mental Capacity Act (MCA) in practice
Supervise, mentor, and support team members

Qualified and experienced Social Worker
Strong background in CHC assessments and reviews
Confident applying the Mental Capacity Act
Experience supervising or mentoring staff
Committed to delivering excellent, person-centred outcomes
