Health and Wellbeing Assessor - Grade 8 (£38,934 - £40,755)
We are seeking a dedicated and compassionate Health & Wellbeing Assessor (HWBA) to assist social workers in empowering and supporting vulnerable adult residents of Croydon. In this role, you will support social workers in providing essential services to vulnerable adults and carers. The Council Leadership strongly supports our statutory functions, including Adult Social Care, especially during these challenging times.
As a HWBA, you will play a critical role in the community by assisting with assessments, developing care plans, and coordinating access to services. You will engage with clients to understand their unique circumstances and needs, helping to empower them on their journey to improved wellbeing. This position requires strong interpersonal skills and a deep understanding of the challenges faced by adults with disabilities. You will work closely with social workers and other professionals to ensure clients receive holistic and effective support.
The ideal candidate will have community adult social care experience, a strong commitment to social justice, and a passion for helping others. This is an exciting opportunity to make a tangible difference in the lives of adults and carers while developing your career in Adult Social Care. The role is primarily case work focused, involving client-facing interactions, community liaison through GP Huddles, and partnership working with health and voluntary sectors. We are enhancing our community engagement strategy with increased community involvement and liaison, providing opportunities to gain knowledge and experience working with vulnerable adults, their families, and developing local networks.
We have an established and supportive team, with low vacancy rates and a preference for permanent staff over agency workers. Our support infrastructure includes a dedicated Training Officer for adult social care, a Principal Social Worker for staff welfare and practice support, and recent upgrades to our electronic recording system.
