Overview
To work within Tobacco Dependency Treatment Services as a Tobacco Dependency Advisor and supported by the Tobacco Dependency Service Lead to develop treatment plans related to tobacco dependency of service users. This includes behavioral support, initiation and review of nicotine replacement therapy +/- E-cigarettes in accordance with trust guidelines. The Tobacco Dependency Treatment Service operates 7 days per week and usually within office hours, with flexibility required according to service demands.
Responsibilities
* Support the day to day running of Acute and Maternity tobacco treatment services to ensure safe, well organized and evidenced based care is offered routinely to all identified smokers and provided to all that access the service. Provide support in a range of hospital based settings.
* Plan, co-ordinate and maintain a caseload for trust employees opting for smoking cessation support via the onsite tobacco dependency team.
* Support the Senior Tobacco Dependency Team in raising awareness of tobacco dependency by utilizing learning resources, supporting national health awareness campaign events and guidance to frontline clinical ward staff and volunteers regarding smoking cessation interventions.
* Deliver the service key performance and reporting indicators for tobacco dependency.
* Promote uptake of individualized tobacco dependency care plans by all identified smokers admitted to hospital overnight or within maternity services.
* Ensure where tobacco dependency is identified, service users are offered treatment plans for temporary abstinence in accordance with the smoke free policy, in alignment with the NHS Long Term Plan.
* Utilize carbon monoxide monitors as part of tobacco dependency treatment plans, interpret and document results, and relay findings to service users for education and motivation.
* Manage caseloads for specialist smoking cessation services open to trust employees via tobacco dependency services.
* Assist with the provision of appropriate pharmacotherapy and with training hospital staff in contact with Tobacco Dependent service users.
* Plan own workload within role boundaries under the guidance of the specialist nurse and public health team. Promote uptake of individualized tobacco dependency care plans for hospital inpatients and maternity services.
* Display analytical and judgement skills in the treatment of tobacco dependency, including consideration of co-morbidities and complex medication plans, and social factors affecting care (e.g., homelessness, travelling communities, prisoners, social services).
* Collect service user data (e.g., using Fagestrom and relevant tools) to enable assessment of individually tailored health plans.
* Audit responsibilities to support service analysis, evaluation and quality improvement initiatives.
* Operate under the appropriate scheme for pharmacotherapy such as NRT and Varenicline where appropriate.
* Monitor, order, collect and record medication stock in line with trust medicines management policy; analyse medication stock levels to provide treatment during stay, discharge or in staff services.
* Assess own ability and know when to involve senior staff; assess potential harm to self or others.
* Plan and deliver interventions for identified target groups; develop the onsite tobacco dependency treatment service.
* Enter relevant information into electronic databases; drive to service locations as required; demonstrate computer literacy and keyboard skills.
* Communicate effectively with patients by telephone and in writing; produce accurate records of activities and work undertaken.
* Support development and dissemination of publicity and materials in collaboration with communications and service teams; ensure data storage and IG compliance.
* Assist with audits, surveys, and research to support improvements in practice.
* Assess and develop treatment plans for patients identified as smokers; provide one-to-one or group support to meet individual requirements; conduct assessments in various settings including patients’ homes where appropriate.
* Communicate smoking-related issues in relation to care packages to patients and staff; report interventions and outcomes to the Tobacco Dependency Specialist or Service Manager.
* Ensure a MECC approach in contacts with patients; develop and maintain relationships with individuals and families to educate and promote change.
* Support individuals to develop and review personal health plans; facilitate transfer of care to community stop smoking services or primary care; signpost to other services as needed.
* Identify and manage risks related to client care; offer therapeutic interventions to support quit attempts; maintain patient confidentiality; hold motivational health conversations.
* Advise patients on stop smoking pharmacotherapy (NRT, Varenicline, E-cigarettes); practice safely within infection prevention and control measures.
* Support training for hospital staff in contact with Tobacco Dependent service users; plan and deliver sessions to identify and engage individuals; help patients develop action plans to sustain behaviour change.
* Plan workload to meet targets; ensure follow-up of patients within guidelines to support quit attempts; assist with pharmacotherapy as appropriate and deliver interventions to target groups.
* Contribute to service development and improvement; participate in CPD; support audits, surveys, and R&D activities as needed.
* Demonstrate duty of care with equipment and resources; manage stock and CO monitor loan/maintenance; safety and risk assessments for equipment.
* Provide cover and training to other tobacco dependency service team members as needed; support for new starters and colleagues during sickness or extended leave.
* Follow policies, engage in discussions on procedural changes, and contribute to monitoring, evaluation and audit of the service.
* Ensure compliance with local and national policies; participate in evaluation of activities and programmes to inform service delivery and development.
* Assist with audits and research and development activities to support practice improvements.
Qualifications and Requirements
* Analytical and judgement skills in tobacco dependency and related co-morbidities; ability to consider social factors in treatment planning.
* Ability to collect and manage data and maintain high standards of documentation and records.
* Professional communication skills with patients, families and staff; ability to work across diverse settings.
* Sound knowledge of pharmacotherapy for smoking cessation (NRT, Varenicline, E-cigarettes) and CO monitoring.
* Willingness to drive to service locations and use relevant ICT systems.
Additional Information
* DBS disclosure - It is the policy of North Tees and Hartlepool NHS Foundation Trust that new staff requiring DBS clearance pay the cost, deducted from first month’s pay. Pre-employment DBS check will be completed.
