Overview
Join Medway Community Healthcare as a registered nurse or paramedic leading a Neighbourhood Nursing Team in Strood. Provide nursing care for patients aged 18 and over who are housebound within the Medway area, delivering personalised care and treatment to those with complex and long-term conditions to help them stay safe and well at home. This role involves professional and legal accountability, working as part of an integrated health care team across professional and organisational boundaries.
Responsibilities
* Lead and manage a Neighbourhood Nursing Team in Strood, aligned to a locality, and manage a caseload to provide high-quality nursing care to patients and families in the community.
* Assess, plan, implement and evaluate care; provide professional and legal accountability for care delivered.
* Collaborate with GPs, other professionals and statutory/voluntary agencies; deliver individualised care across services.
* Operate within a 7am to 7pm service, 365 days a year, with shifts including 7am-3pm, 8:30am-4:30pm and 11am-7pm.
* Maintain and develop the District Nursing qualification / community practitioner pathway or commence training within 1 year of starting; participate in ongoing professional development; contribute to education and mentorship as appropriate.
* Coordinate with the local health and wellbeing objectives of the organisation and ensure the delivery of a high standard of care.
Qualifications and Experience
* Recognised degree in adult nursing; Registered Nurse on Part 1 of the NMC register.
* District Nursing qualification / community practitioner pathway or willingness to commence within 1 year of starting; nurse prescriber or willingness to undertake training.
* Full UK driving license and access to a vehicle for the role.
* Post-graduate qualifications in elderly care, chronic disease management, long-term conditions or rehabilitation; evidence of further study and mentorship or training qualification.
* Postgraduate experience in elderly/rehabilitation/community/long-term conditions; autonomous practitioner experience; experience in community healthcare and high-level clinical assessment of complex patients.
* Experience with enabling technology for patient care; motivational interviewing; change management; education and teaching delivery.
* Knowledge of community healthcare trends, admission avoidance, clinical governance and risk management; ability to practice at an advanced level; experience of lone working and decision making; mentoring students and other professionals.
* Intermediate IT skills (Windows, Internet, Outlook, Excel, EPEX); presentation and teaching skills; research awareness and audit capabilities.
