An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Community Physiotherapist. The post will form part of a seven day service based within one of the Trust's locality teams.
The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face-to-face visits and digital technology.
You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focused rehabilitation and reablement programs, as well as complex discharge planning experience. An understanding of Health and Social care provision and ability to work closely with other organisations and stakeholders is essential.
You will need to demonstrate high standards of evidence based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.
