We are currently recruiting for an Enhanced Practitioner to join our growing multi-disciplinary frailty and visiting service. The role will focus on providing assessment and care for some of our more vulnerable patients with complex health and social needs.
Our ideal candidate will have good assessment skills, recognised training in relevant areas of enhanced practice, and experience in frailty and/or care for patients with complex needs as part of a multi-disciplinary team. While frailty-specific experience is valuable, a willingness to develop knowledge and skills is essential.
We are looking for someone to work 25.5 hours per week. This role supports our initiative to promote proactive models of care, helping keep people well and independent, and to avoid unnecessary hospitalisation. The post holder will be supported by aligned services, including strong GP leadership, primary care mental health team, social prescribers, admin, and reception teams, with regular contact with local community services.
The salary will depend on qualifications and experience.
The post holder will provide holistic care to patients, their families, and carers living with frailty and complex care needs, working within their professional scope of practice either at patients’ homes, in the practice, or via telephone.
They will collaborate with the multi-disciplinary general practice team, ensuring clinical safety and excellence within their scope of practice.
