Senior Community Nurse – NELFT NHS Foundation Trust
Are you a Band 6 or an experienced Band 5 looking for an exciting new opportunity? We are seeking a skilled, motivated, and dynamic Nurse with a passion for high‑quality care and service development.
Responsibilities
* Manage your own clinical caseload of community patients.
* Support HCA/rehabilitation assistants with training, supervision, and joint sessions.
* Provide daily input as part of the Rehabilitation And Frailty Team (RAFT) for up to 21 days in the patient’s own home.
* Work during service hours 8 am – 8 pm, with a care element added 7 am – 10 pm, 365 days a year, including occasional weekends and bank holidays on a rota basis.
* Collaborate closely with other service providers to avoid admission where it is safe, and provide onward referrals to community services including health and social care.
Requirements
Band 6 level or experienced Band 5, with a record of delivering high‑quality community care. A valid driver’s licence and willingness to drive are essential.
Probationary Period
This post will be subject to a probationary period; internal applicants are exempt unless they are already partway through a probationary period or are a bank member of staff.
Induction
New starters join the Trust on the first Monday of each month and undertake a comprehensive induction lasting up to two weeks, including mandatory training, systems training and equipment allocation. The induction takes place at our head office in Rainham, Essex, and provides opportunities to meet the executive team, senior managers, and attend drop‑in sessions on engagement, health and wellbeing, and key processes.
Benefits
High‑Cost Area Supplement – 5% of basic salary (minimum £1,303, maximum £2,198).
