Social Prescriber Role
As a Social Prescriber, you will play a crucial role in connecting individuals with non-medical resources and support within their local community. The successful candidate will aim to improve overall well-being by addressing social determinants of health and fostering community engagement.
Responsibilities include, but are not limited to:
1. Conducting assessments to understand the social, emotional, and practical needs of individuals.
2. Engaging in consultations to develop a holistic understanding of clients' circumstances.
3. Connecting clients with local community resources, services, and activities tailored to their needs.
4. Collaborating with community organisations, social services, and healthcare professionals to build support networks.
5. Developing personalized support plans with clients, setting achievable goals.
6. Providing ongoing guidance and encouragement to help clients reach their objectives.
7. Maintaining relationships with community groups, charities, and organisations to stay informed about available resources.
8. Organising group activities, workshops, and events to promote social connections and community involvement.
Requirements:
* Relevant qualification in Social Prescribing or related field.
* Experience in a similar role, preferably within healthcare or community organisations.
* Strong interpersonal and communication skills.
* Empathy and trust-building abilities with diverse individuals and communities.
* Knowledge of local community resources and services.
* Enhanced DBS check within the last 12 months, preferably on the update service.
